To set up a dental referral consultation, please have your primary veterinarian complete the dental referral form located on our website. We also require your pet’s current medical records, including recent lab work, to be emailed to us at [email protected]. Once we receive the referral form and medical records from your primary veterinarian, a member of our team will contact you within 1 to 2 weeks to schedule the consultation.
